Notes

Would highly recommend 20/20 LS, Multishot wheel and at least 1 source of pierce from items before swapping to LS. 
Need at least one Unset Ring to fit all skill gems. Two would enable Urgent Orders for Enduring Cry but it makes it harder to balance chaos resists.
Prioritize LS, Enduring Cry, Assassin's Mark, and Berserk for gem quality. Go for LS supports after.
Swap Quicksilver for bossing content. Dropping Life Flask is also an option for content like Maven that has prevent heal mechanics. Diamond and Silver Flasks will bolster DPS, Ruby/Topaz/Sapphire will increase survival against respective elements.

Flaying is a cheap annoint that saves us 4 passives. There are better options and the build up points are decent but the passives saved are much needed and hard to replace on a budget.
Corrupt any jewels with %inc max life and attempt to get Corrupted Blood immunity. Crimson jewels have the best chance to roll life. If you can't get one with life, any will work. Use Corrupted Blood/Bleed immunity Life Flask until you have one.
Can drop the Entrench wheel if you get 21% suppression from items and save 3 passives. 
Can drop the Inveterate wheel and one 5% node from the Entrench wheel if you 43% suppression from items and save 5 passives. Move the mastery to the Entrench wheel.
With more passives you can get up to 4 more 6% inc max life nodes from allocated wheels, 2 points for Master of the Arena to help with Blood Rage degen, or boost DPS with a few options:
Dazzling Strikes + Crit Against Blinded Enemies Mastery - 4 passives; more crit + blind
Heartseeker + Crit Multi Mastery - 4 passives; some crit and more crit multi
Lethality - 3 passives; crit and some crit multi
