Leveling
I'm not great at rushing through the campaign but I used splitting steel. If you get the Titucius' Span shield, you can 


With mana reservation implicits, you can anoint disciple of the slaughter instead of charisma.

Panopticon is still the best anoint for single target DPS with ancestral totems but I like the flexibility of not being force to always have the totems placed.

For ailment immunity before grand spectrums, you need to bench craft the betrayal "chance to avoid elemental ailments" suffix onto your boots and body armour. 
I planned the bench craft into the build items since I play more of an SSF-light style and you get an 100% guaranteed chance this way.

Divine Shield
We don't have a lot of ES but it's still an additional 20% life pool. Blood rage degens the ES and only divine shield/energy leech are practical ways to recover it. 
We could optionally turn off blood rage with minimum frenzy charges but lose a lot of attack speed that way.
Energy leech has potential for more damage but the int requirement is high and it wouldn't recover ES as quickly as divine shield can.

Boss Nuke
I included vaal haste but if you have rage on hit gloves berserk is also a possibility.
