Hit enemies, trauma stacks do damage to you. You mitigate some of it, it turns into life regen. Kaom's Spirit gloves turn it into rage regen. 
Enduring cry gives big rage regen boost for 1 sec (cannot be extended).


You survive because of leech and recoup. Being on low life makes you take some of the damage as damage over time, but still have full recoup. 
Recoup and leech is twice more effective because you only need to fill 50% of your life pool.
And of course armour, max all res, molten shell, armour also mitigates ele damage. Berserk gives mitigation as well.

Actum locks your crit chance, so no drawback from Marylene's Fallacy amulet. But assassin's mark also has no effect on crit chance.
Can still use lucky crit mod (belt mod while focused) to get higher effective crit chance. Crit stays the same but if you don't crit - it rolls 1 more time to see if it crits.
No intelligence is countered by elegant hubris timeless jewel (Caspiro). Which also allows to use Blunderbore body armour.

Shrine effect on atlas passive tree also affects Blunderbore, giving you more life, AoE, damage, char side and stun duration, which is important for Boneshatter.
