These are "killed rare or unique recently", upping rage to 30, or enabling detonate dead of scavenging into the full dps (that hits nice and hard, and triggers innervation consistently on t17s)

Swap out innervate for the frost bomb setup for bossing or on enemies with lots of life recovery/regeneration.
