Notes

Main defence Doppelganger + Divine Flesh. You convert half ele dmg to chaos and this dmg resist by chaos res and mod on chest. 
All phys mitigation come from mod on chest, PB, flesh and stone.
Penitent Mask have 0 downside, because you can't go below 0 pdr.

You can't deal dmg without Ambush (crit). So this is 2 button build!!! CDR Of Travel skill from weapons, boots, and passive on tree reduce its cd from 5 sec to 1.65 sec

Replenishing Remedies annoint for 100% uptime on life flask (permanent 3.1k life regen, just click every 4 second :) ) 
Replica Sorrow of the divine is lazy (but good) option to fix all mana costs for triggers and main skill. 

Wither stack comes from withering step. Because of CDR, its 1 sec cd. Wither stack duration 2 sec (+malev skill duration), so if you on close range, it should be 15 stack after first second (but you need do action to remove elusive). 

Craft weapon with Contempt essence or reforge chaos until hit other phys mod. craft Crit multi or phys if need. No point to min max, dmg already here.

Most expensive part is enlighten lvl 3 (4d), 6 link body armor (3.5d), awakened melee phys, void (1.3d each). Other shoud be less <1d (you don't need exact my items like 27 chaos res frac ring, because i have overcap chaos res and some free suffix, tatoo)

You can fix resist or suppress with tatoo

PS 
PoB changed poison dmg calculations 3 times already. It still seems wrong (on 04.08), because second weapon do not affect poison dps. But 2nd weapon should "double" dps, because skill combine both hand dmg into 1 hit. 
And bigger poison should create bigger caustic ground.
